OHIO LAKER BASKETBALL PAYMENTS

We have a new registration and payment system.  When you click the link below you will connect to our JotForm webpage

1. Complete the ONLINE REGISTRATION and PAYMENT.  

You will have two options:  Full Registration or Pay Fees Only.  You have to do option 1 the first time.

Additional payments can be made thru option two.

2. Print and Sign the INSURANCE WAIVER, and

3. Come ready to play dates and times listed on practice page. 

All Payments must be made by March 1st
